A cyber hack into a third-party vendor's email led to the exposure of patient and clinical data, resulting in care delays and clinical workflow disruptions across the health system. 

Allison Viola, Senior Advisor for Health IT at the U.S. Department of Health and Human Services Office of Inspector General, says data collected is too often inaccurate or not reflective of populations' demographics.
