Minnesota Community Healthcare Network, made up of an alliance between Canvas Health, Guild Incorporated, Mental Health Resource, RESOURCE, INC. and Touchstone Mental Health, announced today it has joined with RelayHealth to establish interoperability amongst its campuses.

RelayHealth, McKesson's health information exchange business, provides clinical connectivity technology for organizations across the healthcare industry.

With help from RelayHealth, MCHN will address the need to unite clinical data, manage patient consent and secure interoperability among the individual hospitals.

"Having access to an individual's complete health record during an emergency department visit or other point-of-care can make a difference in decisions made by practitioners and clients," said MCHN Chair Grace Tangjerd Schmitt, in a statement.

"This new capability will provide individuals and care teams with the information they need - helping them to integrate and coordinate services appropriately across care settings," she added.

MCHN believes joining with RelayHealth will support connectivity and interoperability goals across its care settings to aggregate patient information.

The partnership is designed to support the Mission Hennepin Community Collaborative, established by MCHN through the Minnesota Accountable Health e-Health Initiative and the Hennepin County Medical Center and Hennepin Health partnership, to further the group's health information exchange goals.

The Minnesota Department of Health granted MCHN with State Innovation Model funds, partially from a $45 million testing grant in 2013 from the Center of Medicare and Medicaid Innovation to implement the Minnesota Accountable Health Model.

"By establishing interoperability within its community, MCHN is building a foundation for more connected, coordinated care," Ken Tarkoff, senior vice president, general manager, RelayHealth, said in a statement.
