Dell teams with Ensocare on care coordination and integrated discharge management technologies

The deal ties Dell’s population health solutions into Ensocare's cloud service for care coordination, the vendors said.

Dell Services announced a partnership with Ensocare to enable the coordination of discharge management and care transition processes for hospitals.

The cloud-based discharge management platform is designed to help Dell hospital clients improve bed utilization and patient experience while reducing readmissions.

Dell uses what it dubs a population health solutions approach that includes a portfolio of systems, systems integration, hosting and business process outsourcing.

Ensocare’s technology replaces manual processes with the aim of enabling healthcare organizations to transition patients efficiently through care settings.

The cloud service can also capture and analyze data surrounding patient adherence, patterns of readmissions, provider performance and clinical outcomes, Ensocare said, as well as monitor risk factors to allow for as-needed interventions.

Dell Services vice president Sid Nair said that Ensocare’s technologies figure into common themes among its clients: patient engagement, revenue cycle services and population health.
