Population Care Management Symposium at HIMSS17 to address data strategies for value-based care

HIMSS17 will feature the Population Care Management Symposium: Clinical & Business Intelligence Road to Value-Based Care Success.

The day-long pre-conference symposium, slated for Feb. 19, 2017, will cover topics including strategies to identify your system’s patient population, analyzing a variety of payment models, defining a standards-based platform as it applies to healthcare information, and outlining the makeup of a dashboard as it relates to population health management strategies.

Symposium sessions include:

At the Crossroads of Volume and Value. This session will provide current status of the change to value, and an overview of current regulatory, technology, and market drivers impacting key healthcare systems.

How Well Do You Know Your Population’s Health Risk and How to Manage It? Attendees will discuss frameworks to stratify the population according to their level of risk, conditions, gaps and disparities in care and recommend approaches to using these insights to tailor the intervention.

Do You Know What it Really Costs to Deliver Care? How to Optimize Value & Outcomes. This session will describe an approach to measuring the healthcare value and the corresponding costs during the care delivery process, in order to optimize the value.

What is a Standards-based Healthcare Services Platform, and Why Should it Matter to You? The presenter will provide an overview of a vendor-agnostic health care platform and how it facilitates continuity of care across systems and data sharing for analysis and insight.

Building an Effective Dashboard: What Does the Customer Want? What can Clinical and Business Intelligence Provide? This session will provide overall design components and examples by industry leaders in dashboard design and population health management.

Closing Keynote: The Path Forward. Through examples from the recent implementations of State Innovation Model (SIM) initiatives, attendees will learn about provider and payer pitfalls, how to overcome them, and a roadmap to complete the transformation from FFS to VBC.
