ColumbiaDoctors taps HealthGrid's mobile platform to connect with patients

The New York healthcare provider will use the software to enable clinicians to connect with patients via mobile devices.

ColumbiaDoctors, the large New York-based medical practice, said it will integrate the patient engagement platform of HealthGrid’s into its workflow to coordinate care delivery using the vendor’s mobile technology.

Through the integration, ColumbiaDoctors’ more than 1,700 Columbia University Medical Center physicians, surgeons, dentists and nurses can connect with patients through mobile devices to provide care coordination, access to health information, and to enable self-care management.

HealthGrid’s patient engagement platform helps providers activate, acquire and retain patients while complying with regulatory requirements such as Meaningful Use and Medicare’s Merit-Based Incentive Payment System (MIPS).

“HealthGrid’s intelligent patient engagement platform will enable us to stay connected with patients and keep them engaged, informed and supported at all touch points of their care journey,” George Cioffi, MD, president of ColumbiaDoctors, said in a statement.

HealthGrid’s platform will enable ColumbiaDoctors to enhance the patient experience through timely pre-and post-visit communications, and access to care information that promotes collaboration between ColumbiaDoctors’ care teams and care settings.

The platform will also enable mobile check-in to improve office wait times and free up staff for more effective workflow activities, personalized patient education to patients’ mobile devices, and patient feedback through assessments and surveys.
